The film tells the story of events taking place in Georgetown, 15 years after the exorcism of Regan MacNeil in 1973. Father Joseph Dyer and Lieutenant William Kinderman begin to recall memories of Father Damien Karras, and a strange incident occurs in one of the churches that marks the beginning of a series of mysterious murders. All the victims have a strange and suspicious connection to the past, such as the head of Thomas Kintry being replaced with the head of a statue of Christ. This crime and others baffle the investigators, until they discover that these crimes follow the same pattern as an old serial killer named James Venamun.
