In Newark, Dr. Rose Cotter works as a psychiatrist and faces an exceptional day when she receives Laura Weaver, a graduate student who appears to be in a complete breakdown after a traumatic incident she recently experienced. Laura tells Rose that she feels something is following her and appears to her through the faces of people smiling in a terrifying way, then suddenly collapses in front of Rose in a scene that leaves a deep psychological impact on her. From that moment, Rose begins to notice strange behaviors from patients and those around her.
